Chronic Disease Management Programme

If you are a Medical Card holder and have been diagnosed with a Chronic Disease:

  • Type 2 Diabetes,
  • Asthma or COPD
  • Cardiovascular diseases such as
  • Heart Failure,
  • Heart Attack (Angina),
  • Stroke,
  • Irregular Heartbeat (Atrial Fibrillation) 

We will ask if you wish to take part in the the Chronic Disease Management Programme (CDM).

Twice a year we will contact you and invite you in for structured reviews with your doctor or nurse.  The reviews will check your physical health, order bloods if required, review your medication and issue a prescription covering you until your next CDM appointment.

If your GP feels you may be at risk of developing a Chronic Disease they will invite you to attend a scheduled appointment once a year for a Prevention Programme Consultation.

Women's health

Contraception, Mirena Coil and Implanon fitting, Prenatal Care and Fertility advice, Crisis Pregnancy Options, Cervical Smear tests, Menopause and HRT management, Breast Screening and Antenatal and Post Natal Care

Midleton Medical Centre is a member of the HSE Mother and Child scheme which provides free antenatal care to all expecting mothers, a two-week postnatal baby check and a six-week postnatal check for mother and baby.

Please note that attendances

  • that are not antenatal related and
  • attendances post delivery (except 2 week & 6 week check) are not covered under the scheme and incur a fee.

It is advisable that you get the flu vaccine and a pertussis booster if you are pregnant. This will help ensure that mother and baby have a healthy pregnancy and postnatal period.

Our doctors will be happy to advise you about this service in addition to any other issues or concerns you may have regarding your pregnancy.

  • Before 12 weeks: Visit to GP
  • Before 20 weeks: Visit to maternity Hospital / Obstetrician
  • 24 weeks: Visit to GP
  • 28 weeks 1st pregnancy: Visit to maternity Hospital / Obstetrician (in case of first pregnancy)
  • 28 weeks subsequent pregnancy: Visit to GP
  • 30 weeks: Visit to GP
  • 32 Weeks: Visit to maternity Hospital / Obstetrician
  • 34 Weeks: Visit to GP
  • 36 weeks: Visit to maternity Hospital / Obstetrician
  • 37 Weeks: Visit to GP
  • 38 Weeks: Visit to maternity Hospital / Obstetrician
  • 39 weeks: Visit to GP
  • 40 Weeks: Visit to maternity Hospital / Obstetrician

These two checks are covered by the scheme and are free of charge;

  • 2 Weeks postnatal: Visit to GP (for baby)
  • 6 weeks postnatal: N/A (for both mum and baby)
  • Any additional visits will incur the usual fees.

If you have a significant illness, e.g. diabetes or hypertension, you are entitled to 5 additional
visits to the GP.

Care for other illnesses which you may have at this time, but which are not related to your pregnancy, are not covered by the Scheme.

Dr Mary O'Brien is holding a monthly Menopause Clinic.  Please contact reception to make an appointment.

Your initial appointment will be 30 minutes long. You may need to return to get your bloods taken or get a 24 hr Blood pressure monitor.

You will be required to come back in 6 weeks for a review appointment with Dr O'Brien.

Dr. Healy runs a dedicated Well Child Clinic. This is a detailed hour-long developmental screen of your child at any time from birth to six years of age. This assessment will include a full physical screen and developmental milestones checks in areas including Communication, Social/Emotional development, Gross and Fine motor skills.

The aim of the clinic is to screen for developmental delays that may not otherwise come to light until school-age. This allows us to involve the appropriate allied health professionals to maximise your child’s potential. It may also serve to provide reassurance and guidance for worried parents in this busy and challenging stage of your family’s life. This service is also available for patients not registered to the practice.

We recommend purchasing a good thermometer & weighing scales from your local Pharmacy.

General Health Screening

We offer a wide range of general health services including; Bowel screening, ECGs, blood pressures monitors, spirometry (Lung function test), cervical smears, urine tests, swabs and general adult health. These services are provided by all of our doctors. Only the most appropriate tests are ordered, each tailored to the individual patient in all cases.

Your doctor can order a range of blood tests as necessary or indicated to check for diabetes, elevated cholesterol, thyroid issues, low vitamin B12 etc. All blood testing carries a fee for private and GMS patient alike. Our Phlebotomist runs dedicated clinics on Tuesday and Thursday mornings.

Occupational health

The principal role of Occupational Health is the provision of health advice to organizations and individuals to ensure that the highest standards of Health and Safety at work can be achieved and maintained. Our doctors have a wide knowledge of occupational medicine and are experienced in a number of important areas:

Other Services include

A safe and effective way to remove unsightly skin lesions such as warts, verrucae, skin tags and other suitable lesions, as well as treating pre-malignant “sun-damaged” skin in carefully selected patients. All of our doctors can perform cryotherapy. This service is covered by the medical card and some private health insurers.

Dr. Healy provides injection therapy to musculoskeletal complaints.

This service is available for patients referred by their own GP who are not registered with Midleton Medical Centre. If you have private health insurance this may offset the cost of the procedure.

Our practice nurses can assess and treat chronic or acute wounds as well as skin ulceration. Dressings that require a number of nurse’s appointments a week should be made in advance as the nurse can book up quickly.

We provide this service to patients with Haemochromatosis. It involves the removal of excess iron from the body. It is a 30 minute appointment with the nurse, usually carried out in the afternoon. To avail of this service an initial appointment with one of the doctors is required, or a direct referral by a consultant.

Our travel medicine service includes advice on medications, vaccinations along the latest guidelines available and general risks. Please tell the staff at the front desk that this is why you are booking your appointment. Pre-payment for vaccines in advance of ordering is required.

Most vaccinations take some time to take effect so we would advise that you arrange an appointment with one of our doctors to discuss your travel vaccinations at least six weeks before you travel.

If you are ill or injured and are unable to work, the doctor will assess you.  If appropriate they will provide a work certificate and file and illness benefit certificate with the Social Welfare department for the duration of your illness.

Please note that the doctor can not certify you for any treatment or recuperation abroad.  You must obtain medical  certificates in that country.

If you are ill or injured and you are attending the doctor for review, you may be referred to see a Consultant or for a Scan.

To prevent any delay, we recommend you contacting your Private Health Insurer to see what tests, consults or scans you are covered for with your policy.

This way your doctor can immediately refer you for the required treatment on the day of your consultation.
